Conference Registration Fees
Registration fee includes materials, coffee break refreshments & lunches

Early Bird Special	$750	Registration form and payment must be received by 5:00 PM, December 31, 2002
Early Registration	$800	Registration form and payment must be received by 5:00 PM, February 14, 2003
Regular Registration	$850	Registration form and payment received after 5:00 PM, February 14, 2003

If mailing your registration form and payment, please take into consideration the extra time it will take to reach our office. This may affect the registration category you select. In the interests of fairness, those submitting registrations after the stated deadlines without the appropriate fee will be sent an invoice for any outstanding amounts.

Limited Registration
In order to maintain an intimate setting, the number of registration spaces is extremely limited. We expect the conference to fill up well before April 2003, so we recommend that you submit your registration form as early as possible. Submission of a registration form prior to any of the early registration dates noted above does not guarantee admission.

Confirmation
All accepted registrants will be sent a confirmation, by e-mail or fax, of their registration within three weeks of receipt of this form. If submitting your registration after March 1, 2003, please check this website prior to sending in your registration form to ensure that space is still available.

Hotel
The Rimrock Resort Hotel is located on Sulphur Mountain in Banff, Alberta. For guest room reservations, call 1-800-661-1587. Note: For special room rates, please make your reservation before February 24, 2003, and indicate that you are attending the Provincial Health Ethics Network conference.

Cancellations & Substitutions
Cancellations of registration received on or before March 15 will be refunded the registration fee paid less $100. No cancellation refunds will be provided after March 15. Substitutions will be accepted at any time.

Accessibility
The Rimrock Resort Hotel conference facilities and select guest rooms are wheelchair accessible. If you have concerns or questions about the accessibility of the conference, please contact PHEN at 1-800-472-4066.

Financial Assistance
In recognition of the fact that the combined cost of travel, accommodation and registration fee may be prohibitive for some, PHEN, with the support of the Alberta Heritage Foundation for Medical Research, has a limited number of bursaries available for conference registrants. Priority will be given to those traveling long distances to attend. For more information, please contact a PHEN office at 1-800-472-4066.
